How Does A Person Qualify For
Home Care?
Medicare
is the most common payment source for skilled home health services.
Several
other insurance companies use Medicare criteria to decide if someone
qualifies
for home health services. If
you have any doubts if you qualify – call us!
Medicare
has the following requirements for home health care:
- The Patient
must require the
skills of a Nurse, Physical Therapist or the
continuing care of
an Occupational
Therapist. Examples of this would be the need for exercises to restore
function
after a joint replacement surgery, to monitor recovery after a hospital
stay,
or how the Patient is responding to a new medication.
- The
Patient must be ‘homebound’ (please see the question below for a
further
explanation).
- Services
must be ordered by a Physician
- The
services must be
‘reasonable and necessary’ for the Patient's condition

What Does
It Mean To Be Homebound?
The
definition of ‘homebound’ has changed a LOT in recent years, mostly
because
Medicare realized how much money home health services saves them! While
they
require that it take a ‘considerable and taxing effort’ to leave home,
they
also state that it’s fine for ‘homebound’ people to go to get their hair
cut, attend church, and attend other important meetings.
Generally, they look for the following:
-
The Patient must require the
assistance of a person or device to leave the home, here are some
examples (the
people in all of these examples ARE homebound):
o The
Patient may have some steps to
their
home that they can’t manage
unless they hold
onto someone’s arm. They would then need the
assistance of ‘a person’ to leave
home.
o The
Patient may need a walker or cane to walk safely. That would be the device
they need to leave home. That would be the device
they need to leave
home.
o The
Patient may be blind or have some mental impairment so they can’t leave
home
alone. They would then need the assistance of ‘a person’ to leave home.
o The
Patient may need oxygen to leave home. That would be the device
they
need to leave home.
- -OR- leaving
home must make
their symptoms worse. For example, it may make their pain worse, their
heart
symptoms worse, or make them very short of breath. It may just make
them very
fatigued. These people are homebound.
- -OR-
it must be medically
“contraindicated” (a bad idea) to leave home. There are people who
cannot fight
off infections easily (if they are taking chemotherapy for cancer,
etc.), or
who have an open wound. These people are also homebound.

What
Rights Does a Home Care Patient Have?
- Our patient
bill of rights is designed to recognize, protect,
and promote the rights or each patient to be treated with dignity and
respect.
Our Patient or their representative may exercise these rights.
- The services available through
Alpha Omega Home Health, LLC are
Skilled Nursing, Aide, Physical and Occupational Therapy, and Medical
Social
Work. All services are available directly, although some of our
Physical and
Occupational Therapists are contracted through private agencies.
- To have their property treated with
dignity
and respect.

- To be informed of any changes in
the billing
or payment information we have provided within 30 days from the date we
are
made aware of the change.
- The names and professional
qualifications of
the people who provide your care and the proposed frequency of
visits. To participate in the plan for care
and/or any change in
the plan before it is made.
- The Agency’s policy on Advanced
Directives
- To voice grievances regarding
treatment or
care that is (or fails to be) furnished, or regarding the lack of
respect for
property by anyone who is furnishing services on behalf of Alpha Omega
Home
Health, LLC. You will not be subject to discrimination or reprisal for
doing
so. An investigation is made of all complaints, and we document the
existence
of and resolution of the complaint.
- To receive service without regard
to race,
creed, gender, age, handicap, sexual orientation, veteran status,
lifestyle,
marital status, weight or height.
- To receive service without regard
to whether
or not any advance directive has been executed.
- To make informed decisions about
care and
treatment plans and to receive information in a way that is
understandable to
you.
- To be notified in advance of care
to be furnished,
any changes in the care to be provided, treatment options, transfers,
and when
and why care will be discontinued;
- To receive and access services
consistently
and in a timely manner in accordance with Alpha Omega Home Health, LLC
policy;
- To receive education, instructions
and
requirements for continuing care when the services of the Agency are
discontinued. To participate in the selection of options for
alternative levels
of care or referral to other organizations, as indicated by your need
for continuing
care;
- To have your clinical records and
health
information treated in a confidential manner.
- To be advised that Michigan,
Medicare and
Community Health Accreditation Program (CHAP) have toll-free Hotlines
for
questions or complaints about this organization or the implementation
of
Advance Directives:
- The CHAP Hotline number is
1-800-669-1656,
it’s available 24 hours daily.
- The State of Michigan
Complaint Hotline is
1-800-822-6006. The hotline is a 24-hour
service with a person answering the
phone from 8 a.m. to 5 p.m., and an answering
machine after
those hours.
- The Medicare Hotline number is
1-800-447-8477.
Its hours of operation are 8 a.m. to 5 p.m., Monday through Friday. The
purpose
of this hotline is to report fraud, abuse and waste in the
Medicare and
Medicaid programs.
- To receive disclosure information
regarding
any beneficial relationships the organization has that may result in
profit for
Alpha Omega Home Health, LLC.
- To
know that Alpha
Omega Home Health is owned by Joanne Sraj Sopko, RN.
- Patient's
may request information regarding Alpha Omega Home Health’
liability insurance or with any comments, questions, complaints or
compliments
between the hours of 8 a.m. and 4 p.m.

What's The Difference Between Skilled Home Health Care and Private Duty Home Care?
Skilled home health care is
generally for more acutely ill people, generally after discharge from
the hospital or nursing home. The services are directed by the
physician, nurse and any therapists involved (with the patient/family's
input, of course!). Services are 'medically necessary' and usually paid
for by health insurance.
Private duty home care is usually provided to patients whose conditions
are more stable. Typically they need less skilled services, such as
assistance with bathing, dressing, or a nurse to set up their pills for
the week. Insurance companies consider such services 'custodial', so
they are not medically necessary, and are paid for by the
patient/family and some insurance policies purchased specifically for
that purpose.
